What to ask for
The fastest option is usually a Letter of Medical Necessity or a Treatment Summary signed by your provider, listing your active protocol. For many requests, including the VA and other clinics, that's all the other office needs.
If your doctor needs more detail, we can send you your provider clinical notes to forward or bring to your appointment.
How to request your records
Send us a message through your portal or email contact@alphamd.net. Tell us who's asking and what they need, and we'll get the right document to you.
If your doctor's office wants to request them directly
Ask them to go through you instead. If they need a direct release, message us and we'll explain what's needed.
