How long a shipment lasts depends on your dose and how much medication was sent. Your dose sets how much you use each week, and the amount sent sets how many weeks that covers. So two members who started on the same day can have different refill dates.

Your next order date is based on that, not on a fixed calendar. If your next order looks further away than you expected, it's because your current supply still covers you.

Where to find your dates

Your My Orders page shows your next anticipated order under Upcoming Anticipated Orders, along with tracking for orders that have shipped.

Yes. When your medication is getting close to running out, we send your refill automatically. You don't need to request it.

When a refill can't go out automatically

Sometimes something needs to be resolved before we can send your refill. When that happens, we'll let you know weeks in advance, so there's time to sort it out without a gap in your treatment.

The usual reasons:

A missed payment or an outstanding balance on your account

Updated labs are due. A licensed provider needs current results before continuing.

A new consultation is required. In some cases this is a legal requirement.

A survey needs completing, depending on your state's requirements

If you get one of these notices, act on it when it arrives. The sooner it's resolved, the more likely your next shipment arrives on time.

Update it yourself under My Profile in your portal, or email contact@alphamd.net and we'll update it for you.

Please update your address at least 2 weeks before your next order so the change reaches the pharmacy in time.

If your order ships to your old address because we weren't told in time, a $35 reship fee applies.

Shipping needs a physical address. See Can you ship to a PO box?

No. Our pharmacies ship with UPS and FedEx, which can't deliver to PO boxes, so we need a physical address.

If you can't be home to receive a package, see Can I pick up my package at a UPS or FedEx location?

Yes. You can ask the carrier to reroute your package to a pickup location, such as a UPS Access Point or FedEx location. You may need to show ID when you pick it up.

Reroutes are arranged directly with the carrier, and we can't guarantee the carrier will complete one in time. If a reroute doesn't go through and your medication has to be reshipped, a $35 reship fee applies.

Send us a message through your portal or email contact@alphamd.net. Tell us what happened, which medication it was, and how much you have left. If it arrived cracked or damaged, include a photo.

Is there a fee?

Usually. A replacement vial costs $100.

If it arrived broken or damaged in shipping, the replacement is free as long as you send us photos within 48 hours of delivery.

Don't use a vial that's cracked or has a damaged stopper. See My vial says discard after 28 days. Do I have to?

Orders & shipping

After you place and pay for your lab order, Assist Health emails your lab requisition within one business day.

The email comes from Assist Health, not AlphaMD. Check your spam or promotions folder if you don't see it.

Nothing is sent until your lab order is paid. If you haven't paid yet, open the lab order email we sent you and finish checkout.

Wait for the requisition email before you go to the lab.

What should I do when I check in at the lab?

Take your requisition to a free-standing Labcorp. Find one with the Labcorp location finder.

Skip locations inside a hospital, clinic or doctor's office. They bill differently. The location finder doesn't filter them out, so check before you go.

Quest won't accept this requisition. It has to be Labcorp.

Walk-ins are welcome. If you'd rather pick a time, book it yourself on Labcorp's website.

What to bring

Bring only your AlphaMD requisition. Leave any other doctor's lab order at home.

At check-in

Show the requisition with the barcode.

Don't show your insurance card, don't say you're self-pay, and don't pay the lab. Your lab order is already paid.

Ask the lab to remove any insurance they already have on file and to bill the account number printed on your requisition.

After your draw

Your results post to your AlphaMD account automatically. See When will a provider review my lab results?

A licensed provider reviews new results within 1–3 business days of receiving them. You'll get a message with their recommendation, for example no change, a dose adjustment, or when your next labs are due.

How your results reach us

If you used your AlphaMD requisition at Labcorp, your results come to us automatically. You don't need to do anything.

If you're uploading results yourself, use the Files section of your portal.

If you upload results yourself, the report must:

be the official lab report (not a screenshot or summary)

show your name and date of birth

show the date your blood was drawn

What can slow it down

Injection timing. Your provider needs to know how long it was between your last injection and your blood draw. We may contact you to ask, and a quick reply keeps things moving.

An unusable upload, such as a screenshot, a partial report, or a report missing your name, date of birth or draw date.

Missing tests. If a required test wasn't included, you may need additional labs.

Questions about your results?

Reply to the provider's message. Medical questions about your results go to a provider, not general support.

Your first follow-up labs are due about 10 weeks after you start treatment with us. This applies to every member.

We count those 10 weeks from when your first prescription is sent, so the time it takes for your first order to be processed and delivered (usually 7–10 business days) is included. You'll get an email with your lab order when it's due.

You pay for follow-up labs when you place your lab order, and you'll see the price before you pay.

If you were on TRT before joining us

You still need these labs at 10 weeks. Labs from a previous provider can't replace them, however recent. Your medication, dose schedule and injection method often change when you switch to us, and these labs confirm your treatment is working and safe on our protocol.

Once you're stable

When your labs and symptoms are both in a healthy range, your labs move to every 6–12 months. A licensed provider will let you know when your next labs are due.

Assist Health is our lab partner. They're a separate company with partner labs nationwide. Ordering through them gets you discounted pricing, well below what you'd pay walking into a lab on your own.

How your lab order works

A licensed provider orders your labs, during a visit or when reviewing your results.

You get an email with a link to your lab order page. If your labs are due later (for example, at 10 weeks), you'll get a heads-up email now and the lab order email on the due date.

On your lab order page, choose how you'd like to get your tests done.

Pay for your order. Nothing is sent to the lab until you pay. This is the step people most often miss.

Assist Health emails your requisition within one business day. Then take it to a free-standing Labcorp.

Yes. On your lab order page, choose to download your requisition instead of ordering through Assist Health. You can then submit it to your insurance or take it to any lab you like.

Getting your results to us

If you go this way, you're responsible for getting the labs done and getting the results to us:

Have the lab fax them to the number printed on your requisition. Results sent that way reach us automatically.

Or upload them yourself in the Files section of your portal.

If you upload results yourself, the report must:

Results less than 90 days old can be used.

If you live in New York, New Jersey or Rhode Island

Discounted lab ordering through Assist Health isn't available in these states, so this is how you'll get your labs done.

Yes. When you order through Assist Health, you'll see the tests a licensed provider ordered, each with its price. You can add other tests, such as ferritin, IGF-1 or DHT, at the same discounted pricing.

Tests marked required can't be removed. A licensed provider needs those results to continue your care.

Some tests may show as free if they're covered for you.

Your lab order is valid for 30 days.

If it expires before you've paid and had your blood drawn, message us and we'll send a new one.

You can cancel at any time.

Why there may be a balance

Your medication ships several weeks at a time, while your plan is billed monthly. So when you cancel, you may have medication on hand that your monthly payments haven't fully covered yet.

Pharmacy regulations don't allow medication to be returned once it's been dispensed, so you're responsible for the cost of medication that has already shipped to you. Any remaining balance on it is collected before your account closes.

You won't be billed for anything that hasn't shipped.

How to cancel

Your monthly subscription isn't a payment for a box of medication. It covers ongoing access to your care:

licensed providers

the portal where you communicate with them

ordering and shipping of anything they prescribe

Medication is dispensed in batches that last several weeks, so your shipments and your billing don't line up month to month. Some months a shipment arrives and some months it doesn't. Your subscription is the same either way, because your care is continuous.

If you cancel

Because we send several weeks of medication ahead of time, cancelling part-way through can leave you holding medication your subscription hasn't covered yet. In that case, we settle the remaining balance on what has already shipped. See What happens to my balance if I cancel?

The most common reason is the remaining balance on medication you've already received. The same applies if you paused your membership.

Your medication often ships several weeks at a time, while your plan is billed monthly. If you cancel or pause before that medication is fully paid for, the remaining balance is still owed. See What happens to my balance if I cancel?

If your medication was fully paid when you cancelled or paused, you shouldn't see any further charges. If you do, message us and we'll look into it.

Refunds go back to the card you paid with, usually within 3–5 business days.

For charges more than about 3 months old, the card can't be refunded, so we refund you by check instead. We'll let you know when it's sent.

You update your payment method yourself, in your account. For your security, support can't add or change a card for you, and we never take card details by email or portal message.

Set it as your primary form of payment.

Future charges will use your new primary card automatically.

If a payment fails, your plan is paused and your next order won't ship until the payment goes through. You'll get reminders to update your card.

Once you add a new card and set it as primary, the charge retries automatically the next day. When it goes through, your plan is restored. You don't need to do anything else.

How pausing works

The same rule applies as when you cancel: any remaining balance on medication already shipped to you needs to be paid first. Once the medication you have on hand is paid off, your pause takes effect and you won't be charged again until you restart.

When the switch takes effect

By default, your new plan starts on your next billing date. We'll note it on your account and it happens automatically, with nothing more for you to do.

If you'd rather switch right away, you can. The unused part of the month you've already paid for is credited toward your prepaid total, so you don't pay twice for the same days.

12-month (annual): $98.00 per month

6-month: $109.00 per month

With the military and first responder discount:

12-month (annual): $83.50 per month

6-month: $92.87 per month

Prepaid plans are paid upfront for the full term, plus any applicable tax.

When your term ends

About two weeks before your prepaid term ends, we'll reach out to ask whether you'd like to continue on a prepaid plan.

If we don't hear back, you'll move to standard monthly billing. You won't be charged automatically for another full prepaid term.

How to refer someone

Share your referral link. Create your personal link in My Portal and send it to your friend.

When your free month applies

As soon as your friend joins, we add your free month by moving your next billing date back one month. For example, if you'd normally be billed on the 15th of this month, your next charge moves to the 15th of next month.

You'll get an email letting you know the credit was applied.

Can it be combined with other discounts?

Your friend's $30 discount can be combined with their other discounts. For example, a new military member gets 20% off and $30 off their first month.

Other discounts generally don't combine with each other. If you qualify for more than one, you'll get the largest.

How to request a change

Send us a message through your portal or email contact@alphamd.net. Tell us you'd like a dose change and why. For example, your symptoms haven't improved, you have new symptoms, or your goals have changed (including fertility goals).

We also send check-in surveys from time to time, and you can request a dose change there too.

What your provider needs first

Labs from the last 90 days: total testosterone, free testosterone, estradiol, CBC and PSA. If yours are older, we'll order follow-up labs before a change is considered.

A licensed provider then reviews your labs together with how you're feeling. Dose changes are never made from old or unreviewed labs.

If you're new to TRT

Your first dose review happens with your 10-week labs. Your body needs time to settle on your starting dose before a change can be judged fairly.

After a change

Plan on follow-up labs about 8–10 weeks after any dose change, to confirm the new dose is working and safe.

Will it cost more?

Our standard TRT plan is $129/month and covers doses up to 200 mg/week, so most dose changes don't change your price.

Above 200 mg/week, the monthly price is (plus tax):

210 mg/week: $132.75

220 mg/week: $136.50

230 mg/week: $144.00

Every order includes supplies for each injection your medication covers, plus alcohol prep pads. What you get depends on how you inject:

Subcutaneous (under the skin), the most common: 1 mL insulin syringes with the needle attached, so you draw and inject with the same syringe. Typically 29G, ½ inch.

Intramuscular (into the muscle): injection kits with a 21G drawing needle, a 25G injection needle, and a 1 mL syringe.

The exact syringe can vary by pharmacy.

Draw your dose. Your syringe is marked in units: 10 units = 0.1 mL. So a 0.25 mL dose is 25 units, and 0.5 mL is 50 units.

Pinch a fold of skin on your belly or thigh and insert the needle.

Inject slowly, then withdraw.

Don't recap the needle. Put it straight into your sharps container.

How long it lasts depends on your prescribed dose:

Follow the storage directions on your pharmacy label, and don't use it past the discard date on the label.

No. HCG is never billed automatically with your TRT.

Each order is a separate charge that you approve by signing a payment agreement in your portal. When you're running low, message us and we'll set up your next order.

What your provider may do

Prescribe anastrozole to keep on hand. It's an estrogen blocker you take only if you need it. Your provider will tell you how and when.

Order labs to check your estradiol, testosterone and blood counts.

Lower your TRT dose for a while. This is often temporary. Once your labs and symptoms settle, your provider can talk with you about moving back toward your previous dose.

No. Anastrozole is free.

If it's part of your protocol from the start, it ships with your TRT. If it's added later, it ships separately.

Tadalafil (Cialis) and sildenafil (Viagra) are available to members with an active TRT plan. They aren't sold on their own.

Order it from the shop in your portal, or send us a message. We'll ask a few quick health and safety questions, and a licensed provider then reviews and approves your prescription.

Cost Plus Drugs (recommended) is usually the cheapest option.

Create an account at costplusdrugs.com. Your name and date of birth must match your AlphaMD account exactly.

Let us know once your account is set up.

We send your prescription. It usually appears in your Cost Plus account within 24–48 hours.

You pay Cost Plus directly when you place your order.

Your local pharmacy: send us the pharmacy's name, address and phone number. We don't bill insurance, but your pharmacy may.

Refills

Message us when you're running low. We'll ask the same questions and send a new prescription.

There are two separate payments:

$30 to AlphaMD for each prescription, charged before we send it to the pharmacy. This covers your provider's review and the prescription. If you're prescribed both tadalafil and sildenafil, it's $30 for each. Each refill is a new prescription and another $30.

The pharmacy's price for the tablets, which you pay directly to the pharmacy.

A typical prescription is 90 tablets.

At a local pharmacy, you pay the pharmacy's price, or your copay if your insurance covers it.

Yes. Many members split their weekly dose into smaller, more frequent injections, such as two or three times a week, or every day. Your total weekly dose stays the same. It's just divided differently.

How to request it

Send us a message through your portal or email contact@alphamd.net. Tell us how often you'd like to inject and why. For example, you notice ups and downs between injections, or you're having side effects.

A licensed provider will review your request. Knowing the reason helps them set up the best schedule for you and make sure it's safe.

Please don't change your schedule on your own before a provider has reviewed it.

Does it cost more?

No. Your price stays the same, and we'll send extra syringes and supplies for the additional injections at no charge.

Injectable testosterone is dissolved in a carrier oil. The oil used is listed on your vial label.

True sensitivities to carrier oils are rare. Redness, warmth or soreness at the injection site is usually not an oil allergy.

If you have a rash beyond the injection site, hives, swelling or trouble breathing, call 911 or go to the nearest emergency room, then let us know.

Yes. If you have an allergy or simply prefer a different oil, we can switch you to testosterone in MCT oil. You don't need a documented allergy.

MCT oil adds $50/month to your plan, because it's made by a specialty pharmacy.

Why the label says 28 days

Multi-dose vials are required to carry a 28-day discard date after first use. It's a standard labeling rule for this type of vial. It isn't the medication's expiration date.

Can I keep using my vial?

Yes. Your vial is meant to last for the full time it was prescribed for. Our providers consider it safe to use until it's finished, as long as you store it as directed on the label and it looks normal.

When to stop using a vial

Don't use it, and message us, if:

the oil looks cloudy, has particles, or has changed color

the vial is cracked or the stopper is damaged

it's past the expiration date printed on the label (this is different from the 28-day discard date)

Please don't stop TRT suddenly on your own. Send us a message through your portal or email contact@alphamd.net. Let us know you'd like to stop, and a licensed provider will set up a plan to bring your dose down safely.

How tapering works

Your provider will lower your dose a little each week until you're off TRT completely. How long it takes depends on your current dose.

What about my billing?

How the plan works

HCG is added while you're still on TRT, and your TRT dose is gradually lowered over about 10 weeks.

HCG and TRT then stop together, and you start enclomiphene, a daily oral medication.

You stay on a low dose of enclomiphene until conception.

The prescription label on your medication box or vial is your official proof of prescription. It's what you show at TSA and anywhere else you travel. Keep your medication in its original labeled packaging.

Flying with syringes and needles

TSA allows unused syringes and needles in carry-on bags when they're with the injectable medication they're for.

Tell the security officer you're carrying syringes and injectable medication, so they can be inspected separately.

Keep medication and supplies together in the original labeled packaging. The pharmacy label speeds up screening.

Use your carry-on. Checked bags can be lost or exposed to extreme temperatures.

Liquid medication doesn't have to fit the 3.4 oz limit when it's medically necessary, but declare it rather than packing it in your quart bag.

Pack sharps securely so nothing can stick an officer during inspection.

Can I get a Letter of Medical Necessity?

It depends on where you're going. A US prescription doesn't guarantee you can bring testosterone into another country. Testosterone is a controlled substance in many countries, and some are much stricter than the US.

Check your destination's rules first. Contact that country's embassy or consulate well before you travel. Many countries limit you to a personal supply of roughly 30 to 90 days.

Some countries need paperwork in advance. For example, Japan requires an import certificate before you arrive. Thailand, South Korea and Australia also treat testosterone as a controlled substance.

Carry documentation. Bring your labeled medication and a Letter of Medical Necessity. For non-English-speaking countries, consider a translated copy.

Keep medication and sharps easy to reach at customs, since needles get extra attention.

It works for most documentation needs, including HSA/FSA, insurance, international travel, and your own records.

Do I need one to travel in the US?

No. The prescription label on your medication box or vial is your official, legal proof of prescription, and it's what you show at TSA.

Sharing with another doctor or the VA?

Our providers send prescriptions electronically and securely, straight to our partner pharmacy, so there's no paper prescription on our end to send you.

For most needs, a Letter of Medical Necessity or the pharmacy label on your medication is what you need.

If neither will work and you need the actual prescription record, send us a message through your portal or email contact@alphamd.net. We can get a copy of the submitted prescription from the pharmacy.

We send your records to you, and you pass them on to whoever needs them, such as your primary care doctor, a specialist, or the VA. We don't send records directly to other offices.

What to ask for

The fastest option is usually a Letter of Medical Necessity or a Treatment Summary signed by your provider, listing your active protocol. For many requests, including the VA and other clinics, that's all the other office needs.

If your doctor needs more detail, we can send you your provider clinical notes to forward or bring to your appointment.

If your doctor's office wants to request them directly

Yes. Your care continues without interruption, and there's nothing you need to do.

AlphaMD's providers work as a group rather than assigning you to one individual provider. Your consultations, lab reviews and medical questions are handled by the group, so if a provider moves on, your treatment, refills and lab reviews carry on as normal. There's no transfer to complete and no gap in your care.

Your full history, including your labs, protocol and notes, stays in your record for the provider who handles your next review.

If you have a question in the meantime, contact us as usual.

What we have to keep

We can't delete your medical record. As a healthcare provider, we're required by law to keep clinical records, including your labs, prescriptions and provider notes, for a set period after your last treatment. The length is set by state law and is generally five to ten years.

Those records aren't used for marketing and aren't visible in a closed account. We keep them because the law requires it, and so you can request a copy later if you need one, for example for a new provider.

If you'd like a copy of your records before closing your account, ask us and we'll send them to you first.
